Consent form

 

This is a consent for I have made to ensure that any footage used of a person is legally proven if there is any dispute as to the allowance of footage containing themselves. This is integral when including an individual in a documentry so that there would be minimal chance for legal reproductions. 

When writing the content I carried out some further research as to what would need to be included.
